FREE Sports Physicals – MCMH Clinics in Red Oak, Villisca and Malvern will be offering FREE sports physicals!
Please bring a completed Iowa Athletic pre-participation physical exam form with you to your appointment. It can be found at www.mcmh.org, just click the banner ad. It must be signed by your parent or guardian.
Physicals are available for grades 7-12. Vaccines are not included.
