Clarinda Municipal Hospital issued the following announcement.
Clarinda Regional Health Center and our community clinics will be offering FREE sports physicals for any student-athletes over the upcoming summer months!
- Free sports physicals will be offered at all our clinic locations during the months of June, July, and August 2021 by specific providers.
- If your child needs immunizations, medication management, or any personalized medical care, a well-child check should be scheduled first. A sports physical can be completed during this visit as well!
- Please bring a sports physical form with the first page completed and signed by a parent or guardian and if a parent or guardian isn't present at the appointment, please have a signed CRHC Consent to Treat Form completed. This form can be found online at ClarindaHealth.com or a paper copy is available by request.
- Call 712-542-8330 to schedule an appointment at any of our clinic locations in Clarinda, Villisca, or Bedford.
